January 28, 2011 – A worker was fatally injured when he fell 25’ through a skylight at commercial project in Toronto 

February 2, 2011 – A worker sustained fatal injuries when the grooming machine he was using fell through the ice road under construction in Dryden.

 February 9, 2011 – A worker succumbed to serious injuries sustained on February 8, 2011 when he was struck by a car while he was positioning a sign as part of the traffic protection plan at a power line project in Kitchener.

 February 14, 2011 – A worker was fatally injured while attempting to install a steel beam from a ladder at a commercial project in Toronto.  The worker subsequently fell.

February 23, 2011 – A worker was fatally injured when the mobile home he and another worker was setting up fell off of its supports and landed on him in Sioux Lookout.  The other worker was critically injured as well.

 March 9, 2011 – A worker was fatally injured when a precast concrete slab weighing approximately 18,000lbs that was suspended by a crane was uncontrollably lowered onto him at a multi-residential project in Waterloo.

 March 18, 2011 – A worker was fatally injured when the dump truck he was driving was hit by a via train while turning into a project in Belleville.

 March 24, 2011 – A worker was fatally injured when he fell 12 feet off of a scaffold at an arena project in Cayuga.

 May 4, 2011 – A worker was fatally injured and another worker was critically injured when the floor of the house they were working on collapsed pinning the two workers at a project in Ottawa.

 May 31, 2011 – A worker was fatally injured when the mini excavator he was using rolled over crushing him on the project to build a log cabin near South Lake.

 June 1, 2011 – A worker was fatally injured when he was struck by a loader at a road project in Windsor.

 June 24, 2011 – A worker was fatally injured when he was thrown out of a man lift and into a river where he drowned at a project in Thunder Bay

 August 16, 2011 – A worker fell approximately 14 feet off the second storey roof of a house in Toronto while doing roofing work.

August 18, 2011 – A worker was found unconscious under an 8 foot ladder at a construction project in Vaughn.  On August 19, 2011 the worker succumbed to his injuries.

 August 23, 2011 – A worker sustained serious injuries when he was backed over by bobcat crushing his legs at a residential project in London.  The worker succumbed to his injuries on August 24, 2011

 September 8, 2011 – A worker sustained fatal head injuries as a result of a fall at a commercial project in the central region

 September 30, 2011 – A worker sustained fatal head injuries while operating the locomotive inside the tunnel at a tunnelling project in Brampton.

 October 11, 2011 – One worker sustained fatal injuries while another worker received critical injuries and 3 other workers received minor injuries when a caisson drilling rig overturned at the subway tunnel project in Toronto.

 October 12, 2011 – An elevator mechanic received serious injuries resulting in death while in an elevator shaft at a multi residential project in Ottawa.

 October 28, 2011 – A worker receive fatal chest injuries when a wall collapsed during a demolition job in Brampton.

 November 18, 2011 – A worker sustained fatal injuries when he was trapped between a pipe and the guardrail of a elevating work platform at an industrial project in St. Catharines.

 December 20, 2011 – A worker sustained fatal injuries when he fell 2 stories off the roof of a new residential project 35km north of Lindsay.
