January 23, 2004 A worker on a residential project in Tottenham (Central Region) was
performing roof work and fell. The worker succumbed to his injuries on January
31.
February, 2004 No fatalities.
March, 2004 No fatalitles.
April 10, 2004 A worker while installing roof trusses
on a residential project in Burlington fell 9 feet to the second floor.
April 18, 2004 A worker setting up to paint a barn fell
approximately 20 feet on a project in the Windsor district. The worker was not
wearing fall protection.
April 19, 2004 A worker while installing a drainage
system around a basement was buried when the excavation collapsed on a
residential project in York.
April 28, 2004 A worker on a residential project in
Scarborough was struck-by a coffee truck and suffered head injuries.
May 10, 2004 A worker inside a culvert carrying out an
inspection to repair a leak, drown when an inrush of water flooded the
culvert on a project Dutton-Dunwich Township (London South district).
May 17, 2004 A worker on a residential project in
Toronto was knocked of a roof by a sheet of plywood. The worker fell 21
feet sustaining head injuries.
May 28, 2004 A worker working off a pier extracting
water fell into water and drowned on a project in Collingwood.
June 2, 2004 - A worker was electrocuted working
on an electrical system on a project in Ottawa East district.
June 2, 2004 A worker was retrofitting a lighting
system and was electrocuted on a commercial project in Toronto.
June 11, 2004 A worker helping to unload a flatbed
truck was standing on top of the load to be lifted by a forklift. The forklift
operator moved the forklift forward to reach the load with the forks. The forks
struck the load causing the load with the worker to fall off the flatbed
on a project in Pickering. The workers head hit a concrete curb and he
succumbed to his injuries on June 24.
June 14, 2004 A worker was crushed-by apiece of
concrete measuring 1x 4 that was being lifted. A backhoe equipped with forks
was lifting a concrete slab when the slab split causing part of the slab to
fall onto the worker on a project in Uxbridge.
July, 2004 - No fatalities
August 25, 2004 A worker (16 year old) was struck-by
a bulldozer on a project in Hamilton.
September 14, 2004 A worker was struck-by
an asphalt paver and sustained multiple injuries on a project in Hagersville.
The worker succumbed to his injuries on September 17, 2004.
October 19, 2004 A worker while operating an
all terrain track mounted drill rig was crushed-by the rig when it
overturned on a project in Hamilton.
October 25, 2004 A worker was insulating a
single family home in Mallorytown (Kingston district) when apparently he fell
off a ladder and succumbed to his injuries on October 27.
October 29, 2004 A worker fell approximately
22 feet off a second storey balcony while installing siding on a residential
project in Halton. The worker received head injuries.
November 8, 2004 A worker (young worker 18 years old) fell
approximately 20 feet off a roof on a residential project in Milton. The
project consisted of roofing a barn that is being converting into a
single-family home. The worker suffered serious head injuries and succumbed to
those injuries on November 11.
November 30, 2004 A worker (roofer) while access roof
via a ladder fell when the ladder shifted. The worker sustained head
injuries.
December 13, 2004 A worker (structural steel) fell
approximately 40 feet from a rooftop on a renovation project in Mississauga.
December 15, 2004 A worker fell from a ladder on
a project in Toronto and succumbed to his injuries on December 18.
December 16, 2004 A worker (drywaller) fell
approximately 7 feet off an elevating work platform on a commercial project in
Mississauga.
